Your signature below gives Cheer Extremes owners, officers, or employees permission to seek medical treatment for the participant in the event they are not able to reach their parent or guardian.

Fall Session Agreement

**CHEER CLASSES** I understand that I am committed to a Sept 3rd - May 2024-2025 contract with Cheer Extreme. Cheer Extreme Parent/Guardian I will pay monthly tuition and the uniform fee of $200 on time by the specified dates. I understand that tuition is due by the 5th of the month. If tuition is not paid by the 5th and funds aren’t available, you will be charged a $10 late fee. If Uniform Fee is not paid by Oct 1st, you will be required to pay a $20 late fee. I understand that Cheer Extreme does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I will do my part to make sure that my child is to practices and performances in order to show loyalty to my teammates and coaches, and help keep Cheer Extreme a positive and successful program for all involved. I will keep the coaches informed of any absences or illnesses interfering with Cheer Extreme activities.

**TUMBLING CLASSES CONTRACT** I understand that I am committed from Sept. 3rd - May, 2024-2025 contract with Cheer Extreme. I understand that tuition is due by the 5th of the month. Release of Liability * By checking the box, you agree to the terms and conditions set by Cheer Extreme. Release of liability In consideration of allowing the previously-declared participant to begin participation in Cheer Extreme activities, the undersigned, being the legal and acting guardian of participant, acting for themselves and on behalf of the participant, release and hold harmless Cheer Extreme, its owners, officers, employees, and agents of and from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, Covid-19 risk, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises upon which Cheer Extreme is conducted, or any premises under the control and supervision of Cheer Extreme, its owners, officers, employees, or agents or in route to or from any of said premises, or while at any premises or place when activities sponsored by or participated in by Cheer Extreme, its owners, officers, agents, or employees. I understand that cheer is a physical activity with risk of injury and additional possible Covid- 19 risks that, even with the greatest precautions and closest supervision, accidents can and do happen. I release the owners of the building and all teachers and staff of Cheer Extreme from any liability or payment should accident or personal injury occur on the premises of Cheer Extreme Gym, during travel to or from classes, during class, or during the waiting period before or after classes. I agree that this student will conform to the behavioral and safety measures expected. I understand that medical insurance and medical expenses related to any injuries are my responsibility as parent or legal guardian of this participant and that Cheer Extreme will not handle such matters. Further, I agree that this student’s image may be used in print and / or visual media for purposes authorized by Cheer Extreme.

Assumption of Risk *

By checking this box, you agree to the terms and conditions set by Cheer Extreme

Assumption of Risk Participation in physical activities can involve motion, rotation, and height in a unique environment and as such carries with it a certain assumption of risk. There are also possible risks of Covid-19. The undersigned and the participant choose to voluntarily enter upon said premises under the control of Cheer Extreme, knowing their present condition and knowing that said condition may become more hazardous and dangerous during the time the participant or the undersigned is upon said premises. The undersigned and the participant voluntarily assume any and all risks of loss, damage, or injury that may be sustained by the participant and/or the undersigned or any property owner by them while on or upon said premises described above. Cheer Extreme may but shall not be obligated to carry insurance on the participant, and the existence of insurance shall not change, alter, or increase the liability of the business to the participant and the undersigned or affect the terms of this Release.

In checking this box, the person: a) That he/she has read thoroughly and understands completely, the terms of Registration and Release and signs it voluntarily. b) That the person signing as Legal Guardian is, in fact, the true and legal guardian and has the consent of the participant.
